This is a complete photoshop job. you can tell its a fake because the shadow's are all wrong.
This is exactly like that scene in the movie Never Back Down where Max and baja ate a spiked watermelon and then turned the empty shell into a pair of helmete they wore while riding the moped and shouting PEWPEWPEW at all passersby.

I would say that the site of mutilated zombie nurses both excites and pleases me, but that would distract from an otherwise beautifully rendered work of fakery. I first played Silent Hill in the middle of nowhere, Texas. Wild dogs actually roamed the area at night. There's nothing quite like dreaming sirens and children with knives are coming to eat you when you're 12. Fun times.
Anywho, pyramid head needs to get his act together. How does he expect to cut up the bodies if he has this giant metal contraption over his head? How is he supposed to rape and pillage if he can't see what he's supposed to destroy? We should all take pity on Mr. Pyramid Head because its obvious that some evil genius decided to prevent him from expressing the joy within his heart in the only way he knows how: by raping zombie nurses and tearing the skin off of the innocent. Seriously, what act of cruelty is so deserving that a psychopathic serial killer with a giant sword can't enjoy the only thing he knows how to take pleasure in?
