This is a complete photoshop job. You can tell its a fake becasuse the shadowe's are all wrong.
This is exactly like that scene in the movie Never Back down where Max was roaming the sewers under the influence of drugs and fell into a deep pool. He started swimming believing he now had little flipper arms. He swam and swam avoiding all the turds that were floating around. He then glimbed a ladder out of the sewers and emerged on a riverbank where he imagined he was being chased by turtles with big ass hammers.
