This is a complete photoshop job. Yoiu can tell its a fake because the shadow's are all wrong.
This is exactly like that scene in the movie Never Back Down where Max goes out every night beating and robing and raping. When he gfinally getys caught he is subjected to an experimental treatment that makes him ill when he thinks of violence. Later when he gets out, one of his victims remembers him and extracts revenge. This sevee beating removes the effect of the treatment and allows him to resume his old ways.
